Please call our office to get scheduled as a new patient. After completing a phone intake, you will be instructed to complete the necessary forms before your first appointment.

Please note, there are provider specific consent forms. If you are seeing one of the providers with a consent form listed below, you will need to complete the associated consent form before your first appointment.
Completing paperwork digitally:
Patient paperwork may be completed digitally using a PDF editing software, such as Adobe or a free browser based software such as PDF Escape. You may then upload the completed PDF files using the secure upload button at the bottom of this page.
Completing paperwork hard copy:
If you would prefer to fill out the paperwork by hand, you may download and print the forms. You can then fax them to our office at (907) 929-4902 or bring them by in person. Please call ahead if you plan to bring the paperwork in by hand, as we have limited staff in office at this time.
