Step 1: Give Us a Call
Contact our office at (907) 929-4009 to complete a brief phone intake. Our staff will gather some basic information about your needs and what services you're seeking.
Step 2: Provider Review
Based on your intake, we will work to match you with an appropriate provider. Once a provider has reviewed and accepted your request for care, our office will contact you with the next steps.
Step 3: Complete Your Paperwork
After acceptance, we will provide the necessary new patient forms for you to complete. You can also find them on our

Once your paperwork is completed, our office will assist you with scheduling your initial appointment.
